Code of Conduct

The terms of your employment include the minimum standards of behaviour set out in NZQA’s Code of Conduct (PDF, 8.7MB).  Please take the time to read the Code before completing the relevant section on the Declaration and Acceptance of Offer form.

Conflict of Interest

NZQA must be certain that there can be no perception by stakeholders, suppliers or staff of any individual or organisation receiving preferential treatment over any other individual or organisation.  Please read the further information (PDF, 7.4KB), before completing the declaration as part of your signed acceptance.

KiwiSaver

If you wish to make contributions to KiwiSaver, please complete a KS2 Deduction Form (PDF, 95KB), stating what level of contribution you wish to make, and return to us with your signed acceptance.

If you wish to opt out, complete a KS10 Opt-out Form (PDF, 68KB) and return to us on or after day 14 of employment, but on or before day 56 of employment.

NZQA have appointed AXA as our preferred KiwiSaver provider - please refer to their Investment Statement (PDF, 6.3MB).  If you do not currently have a provider in place and wish to join AXA, please complete the enrolment form (PDF, 57KB) and return with your signed acceptance.

IR 330 - Tax Code Declaration

If you are a new Examination Centre Manager, or you need to change the Tax Code we currently hold for you, please complete a IR 330 Tax Code Declaration (PDF, 149KB) and return with your signed acceptance.
