What if I disagree with the factual accuracy of a report?

This section answers these questions:

An organisation may disagree with the factual accuracy of a report received from the Quality Assurance Division. If so, they should in the first instance contact the report's author to resolve the concerns as quickly and informally as possible.

If the organisation and author cannot reach agreement on the factual accuracy of the report, the provider may request a formal review.

Audit reports

QAD has a formal review procedure relating to disagreements over factual accuracy of audit reports. Full information on the grounds for seeking a review and how to apply for it is contained in the Review of Factual Accuracy application pack.

Applications

An organisation may have concerns about the factual accuracy of reports relating to applications, for example registration, course approval or NQF accreditation. If so, they should write to the manager whose contact details appear in the outcome letter, outlining the specific concerns. The manager will review the report and respond in writing with a subsequent decision.

Appeals of review decisions

Where an organisation is still not satisfied after a decision has been formally reviewed or discussed with the relevant manager, it may seek referral to the Deputy Chief Executive of the Quality Assurance Division.

How much does review of factual accuracy cost?

An organisation will only be charged the standard hourly rate to recover costs if the review finds that the original report was factually accurate and no changes were required. If the review finds factual inaccuracies no costs will be incurred by the organisation.
