Documents Required for Your Country

This is the list of documents required for your IQA. Please follow the instructions on the previous page to ensure that you are providing the correct documentation.

Arrange to have the institution that awarded your qualification/s provide the following to NZQA:

Academic transcripts (information) showing full name of qualification holder, full name of awarded qualification, date of award, full name of awarding institution and content of study programme (for all years of study). This must be attested, dated, stamped and signed by the Controller of Exams/State Boards and emailed direct to NZQA from awarding institution (email address:, with subject line to include your NZQA application number).

In addition, you are to send NZQA legible, coloured, scanned and certified digital copies of the following documents:

Qualification documents (for all qualifications to be assessed)

• Qualification award certificate/s
• Qualification transcripts for all years of study

Identity documents

• Passport bio-data page
• Evidence of name change (if required)

Other documents (if required)

• Apprenticeship or trade experience as required
• Translations of all documents
• Completed Skill Shortage form if you are applying under an occupational group on the Skill Shortage Lists issued by Immigration New Zealand (does not need to be a certified copy)
• Completed Teaching Practicum form(s) (does not need to be a certified copy)
