Strategic Purpose Statement |
The purpose of this micro-credential is to empower Kaiāwhina to undertake a comprehensive non-complex individual support plan review for support of an older adult living in the community. Kaiāwhina will complete this micro-credential under guidance from a Registered Nurse (RN).

Graduate profile |
On successful completion of this micro-credential, learners/ākonga will be able to undertake a comprehensive non-complex individual support plan review for support of older adults living in the community.
Kaiāwhina, under the direction and delegation of a RN, will utilise effective communication strategies for understanding individual needs during reviews and document support requirements. Learners/ākonga will have knowledge of: - Mātauranga Māori, Te Ao Māori, Te Whare Tapa Whā and Whānau. - Effective communication strategies including whakawhanaungatanga, to gain insight into a person's needs. - InterRAI tool, Case Mix model of support, and principles of Restorative Care. - Holistic areas of review within a non-complex service review. - Delegation, direction, and escalation. Learners/ākonga will be skilled in: - Applying a non-complex review tool of a support plan. - Creating SMART goals and required documentation. - Applying the Stop and Watch tool for escalation when concerns are identified in the non-complex review process. |

Entry Requirements
It is expected that Kaiāwhina must: 1. Be working with older people within communities under the direction and delegation of a RN. 2. Have completed the New Zealand Certificate in Health and Wellbeing (Level 3) with strands in Healthcare Assistance; Integrated Practice (Family and Whānau Harm); Newborn Hearing Screening; Orderly Services; Support Work; Vision Hearing Screening; and Whānau, Kin and Foster Care (Level 3) [Ref:2470] and/or the New Zealand Certificate in Health and Wellbeing (Social and Community Services) (Level 4) with strands in Community Health Work; Disability Support; Diversional Therapy; Mental Health and Addiction Support; and Whānau, Community and Social Services (Level 4) [Ref: 2992]. 3. Have completed the following unit standards: * Unit standard 23925 - Support, mentor, and facilitate a person to maintain and maximise independence in a health or wellbeing setting. (Level 3, 6 credits). and * Unit standard 26974 - Describe interaction, supports, and reporting for people with dementia in a health or wellbeing setting. (Level 3, 8 credits) or * Unit standard 28563 - Provide person-centred care when supporting a person with early-stage dementia in a health or wellbeing setting. (Level 3, 8 credits) |

1. Apply effective communication strategies to document needs in a support plan, and communicate service allocation outcomes to the person and/or their whanau. 2. Apply risk and escalation procedures whilst working with older adults, and within the scope of practice. 3. Develop a comprehensive non-complex support plan review underpinned by Māori world views. |
